At SOMOS, improving your health and wellbeing is fundamental to our mission. So whether you need help with housing, routine checkups or help with managing a specific medical and/or mental health/substance abuse condition, we’re here to support and navigate the system with you.

Advocacy

For Your Care

We can assist you with making and keeping appointments, helping you learn about the resources and benefits you can get, and coordinate your care with your providers and social supports. Call us at 877-220-4402, Monday through Friday, from 9 a.m. to 5 p.m., to talk to one of our Clinical Coordination Associates.

Care Management Programs

For members with acute, chronic or complex health conditions, SOMOS offers various Care Management programs that can help you improve your quality of life and your health, and manage your condition at home rather than in the hospital setting. These programs are free of charge, and you can choose to participate or not, at any time―your decision will not affect your health plan benefits or coverage.

Are You Eligible?

Our Care Management programs are offered to eligible individuals whose medical and mental health and/or substance abuse needs would benefit from a holistic approach to their care coordination and social needs. To be eligible, an individual must be a member through a SOMOS partner health plan.
