Entering the New Era of Value-Based Care

In 2015, SOMOS Community Care was born with the goal of enabling independent primary care providers to succeed in the NY Medicaid redesign program called DSRIP (Delivery System Reform Incentive Payment Program). As the initial NY DSRIP program ended in 2020, SOMOS Innovation was formed and has been the only Value-Based Payment Innovator in the state to implement full-risk contracts for the Medicaid population. SOMOS Innovation continues to expand our model that empowers physicians to deliver high-quality, holistic care to underserved populations, with a focus on lowering cost, reducing administrative burden, and improving the patient experience.
